Location: Johns Hopkins All Children's Hospital St. Petersburg, FL

The Pharmacy Clinical Coordinator assumes a role in medication management, supporting and participating in the provision of quality patient care, education, and research in the pediatric setting. S/he contributes to the goals of the department by being accountable for the development and maintenance of service line specific drug use policy and evidence based, cost effective medication strategies, as well as providing clinical services to select patient populations. Responsibilities:

• Defines, creates, and maintains at least one practice focus area where pharmacy can positively impact patient care by supporting evidence-based, cost effective therapy in order to provide superior patient outcomes and stewardship of resources
• Supports the prescribing function of the medication use system through review and interpretation of provider orders to ensure the safe and appropriate provision of care to patients
• Assists patients, nurses, physicians, and other health care professionals in the administration of medications to provide the highest quality of clinical outcomes
• Provides clinical monitoring related to the management of drug therapy to optimize patient outcomes and prevent adverse medication events
• Fulfills state and federal regulatory responsibilities as outlined in departmental policies, procedures and guidelines, including required documentation, to ensure that the highest quality standards for pharmacy practice are met
• Demonstrates understanding of safety literature and regulations, reports medication events and near misses and participates in pharmacy huddle and other safety initiatives in order to promote a safe culture
• Assumes a leadership role in the training and ongoing competencies of pharmacy staff, pharmacy residents and students to support the educational mission of JHACH Department of Pharmacy Qualifications:
  • Graduation from an accredited College of Pharmacy with a Pharm.D. or BS
  • Completion of an ASHP accredited residency program or equivalent experience with training in the area of practice
  • Current licensure as a pharmacist in the State of Florida
  • PALS certification preferred; must be obtained within 90 days of hire
  • Demonstrates high level of skills related to teamwork, organization, and prioritization of workflow
  • Displays customer service and interpersonal communication skills required for effective communication with patients and health care providers
  • Demonstrates high level of analytical ability in order to research and evaluate drug information, interpret patient specific data, and resolve problems regarding medication use
  • Comprehensive knowledge of pharmacology, therapeutics, kinetics, therapeutic drug monitoring and literature retrieval and evaluation as it relates to patient care within the area of specialty practice
  • Working knowledge of pharmacoeconomic principles
