Job Description:
Lee Health is seeking a compassionate and dedicated Mental
Health Navigator to join our esteemed network in Southwest Florida.
As a prime figure in our healthcare system, the Mental Health
Navigator will guide and support patients through the complexities
of mental health services, ensuring seamless access to necessary
treatments and resources. This professional will act as a liaison
among mental health professionals, patients, and their families to
facilitate effective communication and enhance patient care quality
within our community-focused health system. Evaluate and assess
patient needs and barriers to care. Develop personalized care plans
in coordination with health care providers. Provide guidance and
information about mental health resources and services available.
Act as an advocate for patients and families within the healthcare
system. Track patient progress and adapt care plans as necessary.
Educate patients and families on mental health conditions and
treatment options. Coordinate with mental health professionals to
streamline care delivery. Strong interpersonal and communication
skills Ability to manage multiple cases efficiently In-depth
knowledge of mental health disorders and treatment methodologies
Proficiency in electronic health record systems Expertise in
patient advocacy and support Organizational and detailed planning
skills Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Certification in Mental Health First Aid preferred
Licensed Clinical Social Worker (LCSW) or similar credentials
advantageous
