What Makes This Role Different - Value-based, full-risk Medicare Advantage model - Patient panel capped at ~450 patients - Focus on prevention, care coordination, and behavior change - Dedicated care teams (MA, Care Coordinator, Care Facilitator) - Emphasis on quality, patient experience, and physician wellness Your Role - Provide comprehensive primary care to older adult patients - Maintain full accountability for clinical outcomes and patient experience - Serve as a health coach, guiding lifestyle and long-term health improvement - Coordinate specialty and hospital care while staying actively involved - Lead and collaborate with an interdisciplinary care team - Supervise and remain accountable for Advanced Practice Providers (APPs) - Partner with medical and operational leadership to meet quality and cost goals Performance Focus (Supported & Measured) - Hospital admissions per thousand - Appointment utilization (18–21 slots per day) - Patient experience (CG-CAHPS) - Closure of clinical care gaps - Medical cost management Qualifications - MD or DO required - Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics - Active or eligible state medical license - Active DEA - Commitment to value-based, team-oriented care Compensation & Benefits - Salary: $250,000 – $306,714 (based on experience) - Comprehensive benefits package - Paid time off and CME - Long-term career growth and leadership opportunities - Predictable schedule and strong work-life balance MASC102
